Personal Deposit Account Application

Are you ready to open a new personal deposit account at F&M Bank? Would you like to move your money from another bank to F&M Bank? F&M Bank has an account that will fit your needs. Not sure what account you want? Click on the following for information about our Checking, Savings, or Certificates of Deposit. Just follow these simple steps to make the process quicker and easier.

Complete the Account Application (fill in online and "submit.")

You can also complete the following pages to help you move your direct deposit, your automatic payment, and to close your current accounts. Once your new account has been opened we will mail any of them that you ask us to!

  1. Redirect Your Automatic Payment – to change payments you make to companies
  2. Redirect Your Direct Deposit – to change your electronic deposit
  3. Account Closing Form – to close your account(s) at another bank
     

(You'll need to allow 3 – 4 weeks before closing your old account(s) so that all of your deposits and payments have time to be switched over.)

Once we receive your Account Application, we will set up your account(s) and notify you when your documents are ready for you to come in and sign and make your initial deposit.* That's how simple it is!

You can also order a Debit Card for your new Checking Account! Just go to the Debit Card page and follow the easy steps. It will be waiting for you when you come in to sign your documents!
